Bucks Frailty Care Consultancy Services Ltd is a consultant-led medical consultancy providing virtual specialist frailty assessments and independent medical advice for adults aged 65 and over across England.

Led by an NHS Consultant Geriatrician, we support healthcare professionals by delivering structured assessments and clear written recommendations to complement existing NHS and community care.

General Practitioners
Care home clinicians and managers
Community nursing teams
Allied Health Professionals
Solicitors and case managers
Families coordinating care for older adults

We do not replace existing clinical teams and do not provide emergency care.

Our Services

Comprehensive Geriatric Assessment (CGA)

A structured virtual assessment covering:

Frailty and functional status
Falls and mobility risk
Cognition and memory concerns
Continence issues
Medication review and optimisation
Advance care planning

A written clinical summary and recommendations are provided within 10 working days and can be shared with the GP or relevant care team (with patient consent).

Short, Focused Consultations

We also offer shorter appointments for specific health-related concerns, suitable for:

Follow-up advice
Single-issue clinical questions
Medication or falls-risk clarification

These appointments:

Are not for emergencies
Do not replace a full CGA

How We Work With You

Consultations are conducted securely online
Advice is evidence-based and clearly documented
Recommendations are designed to support:
- Safer care planning
- Reduced medication-related harm
- Improved communication with families
Clinical responsibility for implementation remains with the GP or usual healthcare provider

Referrals and Booking

Patients or families book appointments directly via our website
Professionals are welcome to:
- Recommend the service to patients or families
- Share relevant background information (with consent)
- Receive copies of written recommendations if requested

GPs and care homes do not book directly at present, but we are happy to collaborate clinically once a consultation has been completed.
