Bucks Frailty Care Consultancy Services Ltd is a consultant-led independent medical consultancy providing specialist frailty advisory assessments and professional guidance for adults aged 65 years and over.
Led by an NHS Consultant Geriatrician, our service provides independent expert opinion, education, and written recommendations to support patients, families, healthcare professionals, and care providers in understanding and managing age-related frailty and associated challenges.
Our assessments are intended to complement, not replace, existing NHS, primary care, community, and care home services.
Working in Partnership
We recognise that every GP practice, care home, and healthcare organisation has unique requirements and priorities. We are happy to discuss your individual needs and explore how our consultancy service may support your residents, patients, families, and staff.
We can provide specialist advisory input regarding:
- Frailty and healthy ageing
- Falls prevention and bone health
- Medication review and optimisation recommendations
- Advance care planning discussions
- General wellbeing and recovery following illness
- Educational sessions for staff, residents, and families
- Participation in multidisciplinary and family meetings where appropriate
We are flexible in our approach and can discuss bespoke consultancy arrangements, pilot projects, educational programmes, and introductory offers tailored to the needs of individual organisations.
Falls Prevention and Bone Health
Independent review of factors that may contribute to falls risk, mobility concerns, and bone health, with practical recommendations for discussion with the patient's usual healthcare providers.
Medication Review and Optimisation Advice
Advisory review of medication regimens, highlighting potential areas for discussion with the patient's GP, pharmacist, or treating clinician.
Advance Care Planning
Supportive discussions regarding future care preferences, advance care planning, and treatment escalation considerations.
Frailty and Recovery
Holistic advisory assessments focused on frailty, functional wellbeing, independence, recovery following illness, and quality of life.
What We Provide
Following consultation, we provide a written advisory report summarising our observations and recommendations.
With the individual's consent, this report may be shared with their GP and other relevant healthcare professionals to support ongoing care and decision-making.
Our reports are designed to:
- Support informed decision-making
- Promote person-centred care
- Facilitate communication between patients, families, and healthcare providers
- Highlight areas that may warrant further consideration by the patient's usual clinical team

Bucks Frailty Care Consultancy Services Ltd provides independent consultancy, advisory, and educational services only.
We do not:
- Provide diagnosis or treatment
- Prescribe medications
- Deliver hands-on clinical care
- Manage ongoing medical conditions
- Replace NHS, primary care, community, or specialist services
- Provide emergency or urgent medical advice
Any recommendations made remain subject to review, acceptance, and implementation by the individual's usual healthcare professionals, who retain full clinical responsibility for patient care.
Referrals and Booking
Consultations may be arranged by individuals, family members, attorneys acting in the person's best interests, care home representatives, case managers, or healthcare professionals where appropriate.
