What we offer
We provide specialist consultant-led frailty advice for older adults through structured virtual consultations. Our focus is on falls assessment and prevention, including bone health assessment, medication safety, and future care planning, supporting patients, families, GPs, and care home professionals.
- Falls Assessment and Prevention Including Bone Health Assessment
A specialist review for older adults at risk of falls, reduced mobility, frailty, or bone health concerns. The consultation focuses on identifying contributing factors such as medications, frailty, and mobility issues, with clear recommendations to support safer ageing and reduce risk.
- Medication Review & Optimisation Advice
A focused consultant-led review of medications in older adults, particularly where there are concerns about side effects, falls risk, dizziness, confusion, or polypharmacy. The aim is to support safer and more appropriate medication use with clear recommendations for the GP or responsible clinician.
- Advance Care Planning & Treatment Escalation Discussion
A structured discussion for older adults and families to explore future care preferences, treatment escalation planning, and resuscitation decisions (including DNACPR/ReSPECT considerations). This supports informed, person-centred decision-making in collaboration with the GP or responsible clinician.
- Frailty and Recovery Review
Best suited for older adults experiencing frailty, functional decline, recurrent hospital admissions, or concerns regarding recovery following illness or hospital discharge.
