1. About Us
Bucks Frailty Care Consultancy Service Ltd is a private medical consultancy offering assessments and expert advice to support the care of older adults. Our services include frailty assessment, continence advice, cognitive review, falls risk evaluation, medication review, and advance care planning discussions, including resuscitation and treatment escalation preferences.
2. Nature of the Service
- Our services are advisory and assessment-based only.
- We do not provide direct treatment, prescribe medication, or deliver personal care.
- All advice and recommendations are documented and shared with clients and/or their care providers, as appropriate.
3. Eligibility for Use
- Clients must be aged 65 or over.
- If the client lacks capacity to participate in the consultation, a next of kin or legal representative must arrange the consultation on their behalf.
- For clients lacking capacity, appropriate consent must be obtained from a legal representative or next of kin with authority to do so.
4. Appointments & Bookings
- All consultations must be pre-booked online via our website or by direct contact at least 48 hours (2 days) in advance.
- Appointments are confirmed upon receipt of payment, unless alternative arrangements have been agreed in writing.
5. Fees & Payment
- Fees are charged per consultation and clearly stated at the time of booking.
- Payment is required in advance unless otherwise agreed in writing.
- Accepted payment methods include PayPal and credit/debit card.
- Appointments may not be cancelled with less than 24 hours’ notice. In such cases, the full fee may still be charged.
6. Client Responsibilities
- Clients or their representatives must ensure that relevant background information, medical history, current medication lists, and basic measurements (e.g., height, weight, and blood pressure) are available at the time of assessment.
- Clients are encouraged to share the outcomes of the assessment with their GP or care provider for continued care and implementation.
7. Confidentiality & Data Protection
- We are committed to protecting client confidentiality in line with the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018.
- Personal information will be stored securely and used solely for the purpose of providing our services.

8. Limitations of Liability
- We accept no liability for outcomes arising from the care delivered by third parties following our consultation.
- While we provide expert advice, the implementation of recommendations is the responsibility of the client and/or their care provider.
9. Complaints & Feedback
- We are committed to delivering a high-quality service.
- If you have a complaint or wish to provide feedback, please contact us in writing at reception@bucksfrailtycare.co.uk.
- We will respond to complaints in line with our Complaints Policy, within 14 days of receiving the complaint. A copy of the policy is available upon request.
10. Changes to Terms
We reserve the right to update these Terms and Conditions. Clients will be notified of any significant changes prior to their next scheduled appointment.
