1. About Us
Bucks Frailty Care Consultancy Services Ltd is a private limited company providing independent, consultant-led medical consultancy and advisory services for adults aged 65 years and above.
Our services include specialist assessment and professional advice relating to frailty, continence, cognition, falls risk, medication review, and advance care planning discussions, including resuscitation and treatment escalation preferences. All services are consultative and advisory only.
2. Nature of the Service and Regulatory Status
· Services are assessment- and advice-based only.
· We do not provide direct medical treatment, prescribe medication, deliver personal care, or provide ongoing clinical management.
· We do not assume clinical responsibility for clients.
· All assessments result in written recommendations intended to support decision-making by clients, families, GPs, and care providers.
Regulatory status:
Bucks Frailty Care Consultancy Services Ltd does not provide regulated activities as defined by the Care Quality Commission (CQC) and is therefore not required to be registered. Clinical responsibility remains with the client’s GP or usual NHS care providers.
3. Eligibility for Use
· Services are available to individuals aged 65 years and over.
· If a client lacks capacity to participate, the service must be arranged by a next of kin or legally authorised representative.
· For clients lacking capacity, appropriate consent must be obtained in accordance with the Mental Capacity Act 2005 and relevant best-interest principles.
4. Appointments and Bookings
· All consultations must be pre-booked via our website or by direct contact at least 72 hours (3 days) in advance.
· Appointments are confirmed upon receipt of payment unless alternative arrangements are agreed in writing.
· Virtual consultations are delivered via Zoom Pro. Clients are responsible for ensuring they have suitable internet access and a private environment.
· We reserve the right to decline or postpone a consultation if insufficient information is provided or if the service requested falls outside our advisory scope.
5. Fees and Payment
· Fees are charged per consultation and are clearly stated at the time of booking.
· Payment is required in advance, unless otherwise agreed in writing.
· Accepted payment methods include credit/debit cards.
· Cancellations made with less than 24 hours’ notice may still be charged in full.
6. Client Responsibilities
Clients or their representatives are responsible for:
· Providing accurate and relevant background information, including medical history and current medications
· Ensuring basic measurements (height, weight, blood pressure) are available where requested
· Sharing the outcomes and recommendations with the client’s GP or care provider for ongoing care and implementation
· Ensuring a private, safe environment for virtual consultations
7. Consent, Confidentiality, and Data Protection
· Clinical consent forms are collected using Microsoft Forms under our Microsoft 365 Business account.
· Written reports and correspondence are stored securely using Microsoft OneDrive and a password-protected business computer.
· Personal data is processed solely for the purposes of delivering consultancy services.

8. Limitations of Liability
· We are not responsible for the implementation of recommendations or outcomes arising from actions taken by third parties, including GPs, care homes, or other healthcare providers.
· Liability is limited to the provision of professional advisory services based on the information provided at the time of assessment.
· Nothing in these Terms excludes or limits liability for death or personal injury arising from negligence where such exclusion is unlawful.
9. Complaints and Feedback
We aim to deliver a high-quality and professional service.
· Complaints or feedback should be submitted in writing to reception@bucksfrailtycare.co.uk.
· Complaints are handled according to our Complaints Policy, and we aim to respond within 14 days. A copy of the policy is available upon request.
10. Changes to These Terms
We reserve the right to update or amend these Terms and Conditions. Clients will be notified of any significant changes prior to their next scheduled appointment.
