What We Offer
We provide structured, consultant-led frailty consultations with a focus on falls prevention, medication safety, and advance care planning. Our advice supports patients and professionals in making informed decisions about complex health needs in older adults.
1. Falls Prevention & Bone Health Review
A specialist review for older adults at risk of falls, reduced mobility, frailty, or bone health concerns. This consultation identifies contributing factors such as mobility issues, medications, and frailty, with clear recommendations to support safer ageing and reduce risk.
2. Medication Review & Optimisation Advice
A focused consultant-led review of medications in older adults where there are concerns about side effects, dizziness, confusion, falls risk, or polypharmacy. The aim is to improve medication safety and reduce treatment burden, with recommendations provided to the GP or responsible clinician.
3.Advance Care Planning & Treatment Escalation Discussion
A structured consultation to discuss future care preferences, treatment escalation planning, and resuscitation decisions (including DNACPR/ReSPECT considerations). This supports informed, person-centred decision-making in collaboration with the GP or responsible healthcare team.

Is this an NHS service?
No. Bucks Frailty Care is an independent private medical consultancy LTD.
We work alongside NHS and community services and do not replace GP or hospital care.
Do you prescribe medication or provide treatment?
No.
We provide medical advice and written recommendations only.
Clinical responsibility for implementing recommendations remains with the individual’s GP or usual healthcare providers.
Is the service CQC-regulated?
No.
Bucks Frailty Care does not provide CQC-regulated activities. The service is advisory in nature and does not require CQC registration.
How are consultations carried out?
All consultations are conducted securely online using encrypted video platforms (e.g. Zoom Pro).
