Care Home Frailty & Falls Education

Consultant-led educational sessions for care homes supporting older adults

Bucks Frailty Care provides educational sessions for care-home staff, helping teams develop their knowledge and confidence in caring for older adults with frailty and complex health needs.

Sessions are delivered by an experienced Consultant Geriatrician and are designed to complement the work of GPs, pharmacists, community teams and other healthcare professionals involved in residents’ care.

Our care-home education service is for staff learning and professional development. It does not provide individual resident consultations, diagnosis, prescribing, treatment or ongoing clinical responsibility.

Educational Topics

Falls Prevention

Falls in older adults are often multifactorial. Educational sessions can help staff understand common causes and practical approaches to falls prevention.

Topics can include:

  • Common causes of falls in older adults
  • Recognising falls risk factors
  • Postural hypotension and dizziness
  • Medication-related falls awareness
  • Mobility and environmental factors
  • Post-fall observations and escalation
  • Bone health and fracture prevention
  • When to seek further healthcare assessment

Frailty and Functional Decline

Frailty can increase the risk of falls, hospital admission and loss of independence.

Teaching can include:

  • Understanding frailty
  • Recognising changes in a resident’s condition
  • Functional decline and recovery
  • Supporting mobility and independence
  • Nutrition and hydration awareness
  • Recognising deterioration
  • Delirium and acute confusion
  • Advance care planning awareness

Medication Safety Awareness

Many older adults take several medicines, and some medications can contribute to dizziness, low blood pressure, sedation, confusion or falls.

Educational sessions can cover:

  • Polypharmacy in older adults
  • Medicines associated with falls
  • Medication-related dizziness and postural hypotension
  • Sedating medicines
  • Recognising possible medication side effects
  • When concerns should be raised with the GP, pharmacist or prescribing clinician

Medication changes and prescribing decisions remain the responsibility of the resident’s usual healthcare professional.
