Bucks Frailty Care provides consultant geriatrician-led online assessments for adults aged 65 and over. The service supports older adults and families with concerns including falls, frailty, medication issues, reduced mobility, bone health and complex medical problems.

A consultant geriatrician can review several interacting health concerns together, including falls, frailty, medication, dizziness, mobility and bone health. The aim is to provide clear, personalised recommendations and help identify appropriate next steps.

Appointments are conducted securely online and include a structured review of your health concerns, medical history and medication. Clear personalised recommendations are provided, with a written summary where appropriate.
