Medication Review & Optimisation
Medicines play an important role in managing health conditions; however, as people age and health needs change, medication regimens may become more complex. In some cases, medicines may contribute to side effects such as dizziness, fatigue, confusion, or increased risk of falls.
Bucks Frailty Care provides advisory and educational input regarding medication burden and potential medication-related issues in older adults.
This service does not prescribe, alter, or discontinue medication but aims to support understanding of medication-related considerations.
This may include:
- General discussion of polypharmacy (use of multiple medications)
- Awareness of potential side effects or interactions
- Consideration of how medications may impact wellbeing or function
- Encouragement of structured medication review with the GP or pharmacist
- Support in understanding treatment purpose and regimen complexity
The overall aim is to support safe and informed discussions with prescribing healthcare professionals.
