Frailty is a term used to describe a state where a person becomes more vulnerable to the effects of illness, injury or other health problems. It can mean that recovery takes longer and that relatively minor illnesses can have a greater impact on independence and day-to-day activities.
Frailty does not affect everyone in the same way. Understanding the factors contributing to frailty can help identify areas where additional support or intervention may be beneficial.
Recovery from illness or hospital admission can take longer for someone living with frailty. Depending on individual circumstances, maintaining appropriate physical activity, good nutrition and hydration, reviewing medicines, addressing falls risks and accessing rehabilitation or community support may help maintain independence and recovery.
Bucks Frailty Care provides consultant-led frailty assessment and specialist advice for adults aged 65 and over.
The consultation considers the individual's medical conditions, medicines, mobility, falls, nutrition, cognition and ability to manage everyday activities, where relevant. The aim is to understand the factors contributing to frailty and provide individualised advice and written recommendations.
Where appropriate, recommendations can be shared with the individual's GP or healthcare team for further assessment, treatment or ongoing support.
Bucks Frailty Care provides an advisory service and does not replace your GP, NHS services or emergency care.
